Home Ward

At Cavenagh Medical, our goal is to help our patients achieve aging in place. We strive to minimise unnecessary trips to the emergency department or needless hospitalisations. Our experienced doctors are equipped to provide hospital-like care to patients while they recuperate at home. Receiving treatment at home will allow patients to reduce risk of hospital acquired infections, minimise stress (especially the elderly when placed in new environments), and avoid sleepless nights and bland food that come with most hospital stays. A Home Ward will allow the patient to feel more at ease by remaining in a familiar environment and it also allows family members to be more involved in the their loved one’s care.

We are able to create a Home Ward by providing specialised care plans tailored to each patient’s individual needs. This can include daily vitals monitoring, daily home nursing care, physiotherapist sessions, regular blood/urine tests, daily doctor video calls and ad hoc doctor house calls as needed. Treatment can take the form of oral medication, IV medication, or even IV fluids. We believe in patient empowerment and take pride in our open communication with patients.

Common conditions include pneumonia (lung infection), cellulitis (skin infection), gastroenteritis (stomach flu), urinary tract infection, post hospitalisation discharge monitoring.

What can be done in a Home Ward?

Daily Nursing Care

Daily home nursing care to assist with vitals monitoring, activities of daily living (ADLs), wound care, urinary indwelling catheters (IDC) care.

Doctor review

A doctor will do daily video teleconsultations to review the patient. If a more detailed assessment is required, the doctor may also offer a house call visit.

Investigations

If required, regular investigations such as blood tests, urine tests, stool tests, nasal swabs can be carried out at home. This allows the team to monitor the patient’s progress.

Treatment

Oral medication, injections, IV medications and IV drips can be administered at home.
